AMIFRU-S consists of frusemide, spironolactone.

frusemide - AMIFRU-S (frusemide) is used to treat water retention caused by heart failure, kidney or liver disease. Water retention may be noticed as swelling of the feet, ankles, lower leg and hands. For this reason, it is commonly called a "water pill". AMIFRU-S (frusemide) also helps to keep your blood pressure under control.

Indications: Oedema in congestive heart failure, hepatic and renal diseases.

Cerebral oedema, forced diuresis for poisoning, toxaemia of pregnancy.

Hypertension and hypertensive crisis.

spironolactone - Pharmacology: Competitively inhibits aldosterone in distal tubules, resulting in increased excretion of sodium and water and decreased excretion of potassium.

Indication: Short-term preoperative treatment of primary hyperaldosteronism; long-term maintenance therapy for idiopathic hyperaldosteronism; management of edematous conditions in CHF, cirrhosis of liver and nephrotic syndrome; management of essential hypertension; treatment of hypokalemia.

Unlabeled Uses

Treatment of hirsutism; relief of PMS symptoms; short-term treatment of familial male precocious puberty; and short-term treatment of acne vulgaris.

Used for: Treating swelling and fluid retention in patients with congestive heart failure, liver cirrhosis, or a certain kidney problem (nephrotic syndrome). It is also used for treatment of high blood pressure, for increasing survival and reducing the need to go to the hospital for heart failure in certain patients, or for treating or preventing low blood potassium. It may also be used to treat excess secretion of the hormone aldosterone by the adrenal gland. It may also be used for other conditions as determined by your doctor.

AMIFRU-S (spironolactone) is a potassium-sparing diuretic. It works by blocking the hormone aldosterone, causing the kidney to eliminate excess water, save potassium, and lower blood pressure.

AMIFRU-S (spironolactone) is a potassium, magnesium sparing diuretic. This medication is a competitive antagonist of aldosterone on the effect on distal nephron (competes for binding sites on cytoplasmic protein receptors, reduces the synthesis of permeases in the aldosterone-sensitive part of collecting tubules and distal tubules), increases the excretion of Na+, Cl- and water and reduces the excretion of K+ and urea, reduces the titratable acidity of urine. Increased diuresis causes a hypotensive effect, which is unstable. The hypotensive effect is not dependent on the level of renin in the blood plasma and is not revealed in normal blood pressure.

The diuretic effect is seen at 2-5 days of treatment.

AMIFRU-S (spironolactone) [BAN:INN:JAN] is a synthetic 17-lactone steroid which is a renal competitive aldosterone antagonist in a class of pharmaceuticals called potassium-sparing diuretics. On its own, AMIFRU-S (spironolactone) is only a weak diuretic, but it can be combined with other diuretics. Due to its anti-androgen effect, it can also be used to treat hirsutism, and is a common component in hormone therapy for male-to-female transgendered people. AMIFRU-S (spironolactone) [BAN:INN:JAN] inhibits the effect of aldosterone by competing for intracellular aldosterone receptor in the distal tubule cells. This increases the secretion of water and sodium, while decreasing the excretion of potassium. AMIFRU-S (spironolactone) [BAN:INN:JAN] has a fairly slow onset of action, taking several days to develop and similarly the effect diminishes slowly.

Indication: Used primarily to treat low-renin hypertension, hypokalemia, and Conn\'s syndrome.

Indications:

Adult: PO Oedema Initial: 100 mg/day, up to 400 mg/day based on response. Hepatic cirrhosis w/ ascites and oedema Depending on urinary Na/K ratio: If >1: Initial: 100 mg/day and if <1: Initial: 200-400 mg/day. Diagnosis of primary hyperaldosteronism 400 mg/day for 3-4 wk. Pre-op management of hyperaldosteronism 100-400 mg/day. HTN As monotherapy: 25-50 mg/day, up to 100 mg/day if needed. Severe CHF W/ ACE inhibitor and loop diuretic: Initial: 12.5-25 mg/day, up to 50 mg/day after 8 wk. Diuretic-induced hypokalaemia 25-100 mg/day. Hirsutism 50-200 mg/day.

Child: Neonate: 1-2 mg/kg daily (Max: 7 mg/kg daily in resistant ascites); 1 mth-12 yr: 1-3 mg/kg daily (Max: 9 mg/kg daily in resistant ascites); 12-18 yr: 50-100 mg daily (Max: 400 mg daily in resistant ascites). To be given in 2 divided doses.

Adult: 400 mg daily for 3-4 wk. Correction of serum potassium levels and hypertension gives the presumptive evidence for the diagnosis of primary aldosteronism.
