Advantan Milk - Pharmacology: Depresses formation, release, and activity of endogenous mediators of inflammation, including prostaglandins, kinins, histamine, liposomal enzymes, and complement system. Modifies body's immune response.

Indication: Replacement therapy in primary or secondary adrenal cortex insufficiency; treatment of congenital adrenal hyperplasia, hypercalcemia associated with cancer, and/or nonsuppurative thyroiditis; adjunctive therapy for short-term administration in rheumatic disorders; exacerbation or maintenance therapy in collagen diseases; treatment of dermatologic diseases; control of allergic states or allergic and inflammatory ophthalmic processes; management of respiratory diseases; treatment of hematologic disorders; palliative management of neoplastic diseases; management of cerebral edema associated with primary or metastatic brain tumor, craniotomy, or head injury; induction of diuresis in edematous states (from nephrotic syndrome); management of critical exacerbations of GI diseases; management of acute exacerbations of multiple sclerosis; treatment of tuberculous meningitis; management of trichinosis with neurologic or myocardial involvement.

Intra-articular or soft tissue administration

Adjunctive therapy for short-term administration in synovitis of osteoarthritis, rheumatoid arthritis, bursitis, acute gouty arthritis, epicondylitis, acute nonspecific tenosynovitis, and posttraumatic osteoarthritis.

Intralesional administration

Management of keloids; treatment of localized hypertrophic, infiltrated inflammatory lesions of lichen planus, psoriatic plaques, granuloma annulare, lichen simplex chronicus; treatment of discoid lupus erythematosus; necrobiosis lipoidica diabeticorum; alopecia areata; cystic tumors of aponeurosis or tendon.

Unlabeled Uses

Reduction of mortality in severe alcoholic hepatitis; prevention of respiratory distress syndrome; treatment of septic shock; improvement of neurologic function in acute spinal cord injury.

Used for: Treating severe allergies, arthritis, asthma, certain blood disorders, and skin conditions. It may also be used for other conditions as determined by your doctor.

Advantan Milk is a corticosteroid. It works by modifying the body's immune response to various conditions and decreasing inflammation.

Advantan Milk and its derivatives, Advantan Milk sodium succinate and Advantan Milk acetate, are synthetic glucocorticoids used as antiinflammatory or immunosuppressive agents.

Advantan Milk helps relieve inflammation and itch that may occur with certain skin problems and allergies. It is also used to relieve pain in arthritic joints, fluid retention in head injuries, treat inflammatory bowel disease and to control asthma. Advantan Milk may also be used to prevent organ transplant rejection after a transplant. Advantan Milk may be used for other purposes as decided by your doctor. Advantan Milk belongs to a group of medicines called steroids.

Oral Anti-inflammatory or immunosuppressive

Adult: Initially, 2-60 mg/day in 1-4 divided doses, followed by gradual reduction in dosage to the lowest possible level consistent with maintaining an adequate clinical response.

Child: As sodium succinate: 0.5-1.7 mg/kg/day or 5-25 mg/m 2 /day in divided doses every 6-12 hr; "Pulse" therapy: 15-30 mg/kg/dose over ≥30 min given once daily for 3 days. Intramuscular Anti-inflammatory or immunosuppressive

Adult: As sodium succinate: 10-80 mg/day once daily; As acetate: 10-80 mg every 1-2 wk.

Child: As sodium succinate: 0.5-1.7 mg/kg/day or 5-25 mg/m 2 /day in divided doses every 6-12 hr; "Pulse" therapy: 15-30 mg/kg/dose over ≥30 min given once daily for 3 days. Intravenous Anti-inflammatory or immunosuppressive

Child: As sodium succinate: 30 mg/kg over 15 min, followed in 45 min by a continuous infusion of 5.4 mg/kg/hr for 23 hr. Intravenous Lupus nephritis

Adult: As sodium succinate: High-dose "pulse" therapy: 1 g/day for 3 days.

Child: Children: As sodium succinate: 30 mg/kg over ≥30 min every other day for 6 doses. Intravenous Aplastic anaemia

Adult: As sodium succinate: 1 mg/kg/day or 40 mg/day (whichever dose is higher), for 4 days. After 4 days, change to oral and continue until day 10 or until symptoms of serum sickness resolve, then rapidly reduce over approx 2 wk. Intravenous Pneumocystis (carinii) jiroveci pneumonia in patients with acquired immunodeficiency syndrome (AIDS)

Adult: As sodium succinate: 30 mg bid for 5 days, then 30 mg once daily for 5 days, then 15 mg once daily for 11 days. Intravenous Life-threatening shock

Adult: As sodium succinate: 30 mg/kg initially and repeated every 4-6 hr if needed or 100-250 mg initially and repeated at 2- 6-hr intervals as required by direct IV inj over a period of 3-15 min. Alternatively, following the initial dose by direct IV inj, 30 mg/kg by slow continuous IV infusion every 12 hr for 24-48 hr. Intravenous Croup

Child: 1-2 mg/kg followed by 0.5 mg/kg every 6-8 hr. Intra-articular Anti-inflammatory or immunosuppressive

Adult: As 0.1% aceponate ointment, fatty ointment or cream or lotion: Apply a thin coating once daily to the affected area. Duration: <12 wk.

Child: >4 mth: As 0.1% aceponate ointment, fatty ointment or cream: Apply a thin coating once daily to the affected area; Lotion: Apply sparingly once daily to the affected area and rub gently. Duration: <4 wk. Reconstitution: Standard diluent: 40 mg/50 ml D 5 W; 125 mg/50 ml D 5 W. Min volume: 50 ml D 5 W. Incompatibility: Y-site incompatibility: Allopurinol, amsacrine, ciprofloxacin, propofol, sargramostim, vinorelbine, docetaxel, etoposide phosphate, filgrastim, gemcitabine, ondansetron, paclitaxel. Syringe incompatibility: Doxapram. Admixture incompatibility: Calcium gluconate, glycopyrrolate, regular insulin, penicillin G sodium, metaraminol, nafcillin.

Acute spinal cord injury As Na succinate: 30 mg/kg, followed in 45 mins by a continuous infusion of 5.4 mg/kg/hr for 23 hr. Lupus nephritis As Na succinate: High-dose "pulse" therapy: 1 g/day for 3 days. Aplastic anaemia As Na succinate: 1 mg/kg/day or 40 mg/day (whichever dose is higher) for 4 days. Thereafter, change to PO and continue until day 10 or till symptoms of serum sickness resolve, then rapidly reduce over approx 2 wk. Pneumocystis (carinii) jiroveci pneumonia in patients with AIDS As Na succinate: 30 mg twice daily for 5 days, then 30 mg once daily for 5 days, then 15 mg once daily for 11 days. Life-threatening shock As Na succinate: Initial: 30 mg/kg, repeat 4-6 hrly if needed. IV/IM Anti-inflammatory or immunosuppresive As Na succinate: 10-40 mg via IV inj, may repeat via IV/IM inj at intervals based on clinical response. Up to 30 mg/kg may be used if needed; may repeat 4-6 hrly for 48 hr. IM Anti-inflammatory or immunosuppressive As Na succinate: 10-80 once daily. As acetate: 10-80 mg every 1-2 wkly. Intra-articular Anti-inflammatory or immunosuppresive As acetate: Large joints: 20-80 mg; Small joints: 4-10 mg. Give doses 1-5 wkly. Intralesional Anti-inflammatory or immunosuppressive As acetate: 20-60 mg 1-5 wkly. Topical Corticosteroid-responsive dermatoses As 0.1% aceponate oint/cream/lotion: Apply once daily for <12 wk.
