Apirofeno - Pharmacology: Decreases inflammation, pain, and fever, probably through inhibition of cyclooxygenase activity and prostaglandin synthesis.

Indication: IV : In adults for the management of mild to moderate pain and as an adjunct to opioid analgesics in the management of moderate to severe pain. PO : Relief of signs and symptoms of rheumatoid arthritis and osteoarthritis, mild to moderate pain, primary dysmenorrhea, reduction of fever, migraine.

Used for: Treating rheumatoid arthritis, osteoarthritis, menstrual cramps, or mild to moderate pain. It may also be used for other conditions as determined by your doctor.

Apirofeno is an NSAID. Exactly how it works is not known. It may block certain substances in the body that are linked to inflammation. NSAIDs treat the symptoms of pain and inflammation. They do not treat the disease that causes those symptoms.

NSAIDs, a derivative of phenylpropionic acid, Apirofeno has anti-inflammatory, analgesic and antipyretic effect.

The mechanism of action is associated with inhibition of COX activity - the main enzyme metabolism of arachidonic acid, which is a precursor of prostaglandins, which play a major role in the pathogenesis of inflammation, pain and fever. Analgesic effect is due to both peripheral (indirectly, through suppression of prostaglandin synthesis), and a central mechanism (due to inhibition of prostaglandin synthesis in the central and peripheral nervous system). Inhibits platelet aggregation.

For topical use anti-inflammatory and analgesic action. Reduces morning stiffness, increased the amount of motion in joints.

Apirofeno is a nonsteroidal anti-inflammatory agent (NSAIA) or nonsteroidal anti-inflammatory drug (NSAID), with analgesic and antipyretic properties. Apirofeno has pharmacologic actions similar to those of other prototypical NSAIAs, which are thought to act through inhibition of prostaglandin synthesis.

Oral Pain and inflammation

Adult: 1.2-1.8 g/day in divided doses.

Maintenance: 0.6-1.2 g daily. Max: 2.4 g per day. Oral Juvenile idiopathic arthritis

Child: ≥3 mth and weighing >5 kg: 30-40 mg/kg/day in 3-4 divided doses. Max: 2.4 g/day. Oral Fever

Adult: 200-400 mg every 4-6 hr. Max: 1.2 g/day.

Child: 1-6 mth: 5 mg/kg 3-4 times daily, 6-12 mth: 50 mg 3 times daily, 1-2 yr: 50 mg 3-4 times daily, 2-7 yr: 100 mg 3-4 times daily, >7 yr: 200 mg 3-4 times daily. Max: 40 mg/kg/day. Intravenous Closure of patent ductus arteriosus

Child: Given as three IV doses infused over 15 minutes at 24-hr intervals. Initially 10 mg/kg followed by two doses of 5 mg/kg. A second course may be given if ductus remains open after 48 hr. Surgery may be required if neonate is unreponsive to 2 courses of treatment. Dose should be based on birth weight. Topical/Cutaneous Pain and inflammation associated with musculoskeletal and joint disorders

Adult: As 5% or 10% gel: Apply onto affected area. Special Populations: Dose reduction in renal and hepatic impairment. Patients who developed severe mucositis during the 1st course of therapy: Reduce the 2nd course dose to 25%.

Oral Mild to moderate painful conditions and inflammation

Adult: 1.2-1.8 g/day in divided doses.

Maintenance: 0.6-1.2 g daily. Max: 2.4 g/day. Oral Juvenile idiopathic arthritis
