Chininsulfat - Chininsulfat is used to treat malaria, a disease caused by parasites. Parasites that cause malaria typically enter the body through the bite of a mosquito. Malaria is common in areas such as Africa, South America, and Southern Asia.

Chininsulfat will not treat severe forms of malaria, and it should not be taken to prevent malaria. Chininsulfat also should not be taken to treat or prevent night-time leg cramps. Using this medication improperly or without the advice of a doctor can result in serious side effects or death. Chininsulfat is approved for use only in treating malaria. Some people have used Chininsulfat to treat leg cramps, but this is not an FDA-approved use.

The U.S. Food and Drug Administration has banned the sale of all non-approved brands of quinine. As of December 2006, Qualaquin is the only brand of Chininsulfat that is approved by the FDA.

Chininsulfat may also be used for purposes not listed in this medication guide.

Chininsulfat is used parenterally to treat life-threatening infections caused by chloroquine-resistant Plasmodium falciparum malaria. Chininsulfat acts as a blood schizonticide although it also has gametocytocidal activity against P. vivax and P. malariae. Because it is a weak base, it is concentrated in the food vacuoles of P. falciparum. It is thought to act by inhibiting heme polymerase, thereby allowing accumulation of its cytotoxic substrate, heme. As a schizonticidal drug, it is less effective and more toxic than chloroquine. However, it has a special place in the management of severe falciparum malaria in areas with known resistance to chloroquine.

Chininsulfat is used to treat malaria. It may also be used to treat night-time leg cramps and a parasitic infection called babesiosis.

Oral Malaria

Adult: As sulfate: 648 mg given every 8 hr for 7 days.

Child: 10 mg/kg given every 8 hr for 7 days. Renal impairment: Severe chronic renal failure (as sulfate): 648 mg followed 12 hr later by maintenance doses of 324 mg every 12 hr. Oral Nocturnal leg cramps

Adult: 200-300 mg once at night. Oral Babesiosis

Adult: As sulfate: 650 mg every 6-8 hr. To be taken with clindamycin for 7-10 days.

Child: 8 mg/kg (up to 650 mg) every 8 hr. To be taken with clindamycin for 7-10 days. Intravenous Malaria

Adult: As dihydrochloride: Initially, 20 mg/kg (max: 1.4 g) given over 4 hr. Start maintenance doses 8 hr after the start of the initial infusion.

Maintenance: 10 mg/kg (up to 700 mg) given over 4 hr every 8 hr. Loading dose should not be given if patient has received quinine, quinidine, mefloquine or halofantrine during the previous 24 hr. If parenteral treatment is required for >48 hr, maintenance dose should be reduced to 5-7 mg/kg.

Indications: Malaria:a) Uncomplicated resistant faciparum malaria.

b) Complicated and severe malaria including cerebral malaria.

Nocturnal muscle cramps.
