Daptomycin - Daptomycin is an antibiotic that fights bacteria in the body.
Daptomycin is used to treat bacterial infections of the skin and underlying tissues.
Daptomycin may also be used for purposes not listed in this medication guide.
Pharmacology: Binds to bacterial membranes and causes a rapid depolarization of membrane potential, which inhibits protein, DNA, and RNA synthesis, resulting in bacterial cell death.
Indication: Treatment of complicated skin and skin structure infections caused by susceptible strains of gram-positive microorganisms; treatment of Staphylococcus aureus bloodstream infections (bacteremia), including those with right-sided infective endocarditis caused by methicillin-susceptible and methicillin-resistant isolates.
Treatment of vancomycin-resistant Enterococcus faecium.
Intravenous Complicated skin and skin structure infections
Adult: Infections caused by susceptible Gram-positive bacteria: 4 mg/kg once daily given by IV infusion over 30 min for 7-14 days. Renal impairment: CrCl<30 mL/min (including haemodialysis or CAPD): 4 mg/kg once every 48 hr. Haemodialysis patients: Administer dose on haemodialysis (following procedure) whenever possible. Intravenous Staphylococcus aureus Bacteraemia
Adult: Including right-sided endocarditis caused by methicillin-susceptible or methicillin-resistant isolate: 6 mg/kg once daily given by IV infusion over 30 min for 2-6 wk. Renal impairment: ClCr<30 mL/min (including haemodialysis or CAPD): 4 mg/kg once every 48 hr. Haemodialysis patients: Administer dose on haemodialysis (following procedure) whenever possible. Reconstitution: Reconstitute with 10 ml of 0.9% sodium chloride inj using aseptic technique. Gently rotate the vial to wet the powder and allow it to stand for 10 min. To obtain a completely reconstituted solution, gently swirl or rotate the vial. Do not shake/agitate vial during and after reconstitution. Further dilute the reconstituted solution with 0.9% sodium chloride inj for IV infusion. Incompatibility: Compatible with 0.9% sodium chloride inj and lactate Ringer's inj. Incompatible with dextrose-containing diluents.
Adult: IV Complicated skin and skin structure infections 4 mg/kg/day infused over 30 min for 7-14 days. Stapylococcus aerues bacteraemia ; including right-sided endocarditis caused by MSSA or MRSA 6 mg/kg/day infused over 30 min for 2-6 wk.
