Dofetilide - Pharmacology: Blockade of the cardiac ion channel carrying the rapid component of the delayed rectifier potassium currents.

Indication: Maintenance of normal sinus rhythm (delay in time to recurrence of atrial fibrillation/atrial flutter [AF/AFl]) in patients with AF/AFl of more than 1 wk duration who have been converted to normal sinus rhythm; conversion of AF/AFl to normal sinus rhythm.

Dofetilide is a heart rhythm medication, also called an antiarrhythmic.

Dofetilide is used to help keep the heart beating normally in people with certain heart rhythm disorders of the atrium (the upper chambers of the heart that allow blood to flow into the heart). Dofetilide is used in people with atrial fibrillation or atrial flutter.

Dofetilide may also be used for purposes not listed in this medication guide.

Dofetilide is an antiarrhythmic drug with Class III (cardiac action potential duration prolonging) properties and is indicated for the maintenance of normal sinus rhythm. Dofetilide increases the monophasic action potential duration in a predictable, concentration-dependent manner, primarily due to delayed repolarization. At concentrations covering several orders of magnitude, Dofetilide blocks only IKr with no relevant block of the other repolarizing potassium currents (e.g., IKs, IK1). At clinically relevant concentrations, Dofetilide has no effect on sodium channels (associated with Class I effect), adrenergic alpha-receptors, or adrenergic beta-receptors.

Oral Atrial fibrillation and atrial flutter

Adult: 500 mcg bid if corrected QT interval is ≤440 milliseconds, measure QT interval 2-3 hr after the 1st dose. Reduce maintenance dose if QT interval becomes prolonged after the 1st dose. Stop treatment if QT interval exceeds 500 milliseconds. Treatment must be initiated under ECG monitoring and continued for at least 3 days. CrCl (ml/min) Dosage Recommendation 40-60 250 mcg bid. 20-39 125 mcg bid. <20 Avoid use.

Oral Highly symptomatic atrial fibrilation or flutter

Adult: 500 mcg bid if corrected QT interval is ≤440 milliseconds; reduce maintenance dose if QT interval becomes prolonged after the 1 st dose. Stop treatment if QT interval exceeds 500 milliseconds.
