Pilocarpine - Used for: Treating dry mouth associated with radiation treatment for cancer or Sjogren syndrome. It may also be used for other conditions as determined by your doctor.

Pilocarpine is a cholinergic agent. It works by increasing the secretion of saliva from the salivary glands, which helps to relieve dry mouth.

Pilocarpine is an agent that affects the nervous system. Pilocarpine increases saliva secretion in the mouth.

Pilocarpine is used in the treatment of dry mouth.

Pilocarpine may also be used for purposes other than those listed in this medication guide.

Pharmacology: Ophthalmic

Decreases IOP by constricting pupil and stimulating ciliary muscles to open trabecular meshwork spaces and facilitate outflow of aqueous humor.

PO

Stimulates exocrine glands including mucous cells of respiratory tract and salivary glands in oral cavity.

Indication: Ophthalmic gel

To control IOP.

Ophthalmic solution

Management of glaucoma, especially open-angle glaucoma; management of acute (closed-angle) glaucoma alone and in combination with other cholinergic agent or carbonic anhydrase inhibitors.

Treatment of xerostomia (dry mouth) in patients with malfunctioning salivary glands because of radiotherapy for cancer of head and neck; relief of dry mouth in patients with Sjögren syndrome.

Unlabeled Uses

Relief of dry mouth in patients with graft-vs-host disease (PO); keratoconjunctivitis sicca (dry eye syndrome).

Pilocarpine is a choline ester miotic and a positively charged quaternary ammonium compound. Pilocarpine, in appropriate dosage, can increase secretion by the exocrine glands. The sweat, salivary, lacrimal, gastric, pancreatic, and intestinal glands and the mucous cells of the respiratory tract may be stimulated. When applied topically to the eye as a single dose it causes miosis, spasm of accommodation, and may cause a transitory rise in intraocular pressure followed by a more persistent fall. Dose-related smooth muscle stimulation of the intestinal tract may cause increased tone, increased motility, spasm, and tenesmus. Bronchial smooth muscle tone may increase. The tone and motility of urinary tract, gallbladder, and biliary duct smooth muscle may be enhanced. Pilocarpine may have paradoxical effects on the cardiovascular system. The expected effect of a muscarinic agonist is vasodepression, but administration of Pilocarpine may produce hypertension after a brief episode of hypotension. Bradycardia and tachycardia have both been reported with use of pilocarpine.

Pilocarpine controls production of fluids by the salivary and lacrimal glands and is used because of that property for a long time to treat glaucoma. By reduction of fluid the eye pressure drops thus stopping the symptoms of Glaucoma development.

  • Pilocarpine is used to treat severe dry mouth in people who have received radiotherapy treatment for head or neck cancer. It is also used to treat severe dry mouth and dry eyes in people who have an autoimmune condition called Sjogren's Syndrome. Pilocarpine helps to relieve your discomfort by helping you produce more saliva and tears. Pilocarpine cannot cure your condition, it can only help to relieve your symptoms of dry mouth and dry eyes.

    Pilocarpine is used to treat certain types of glaucoma and other causes of high pressure in the eye. Pilocarpine helps to lower the pressure in the eye. If left untreated, an increase in pressure within the eye can cause loss of vision and damage to the eyes. Pilocarpine may also be used before or during eye surgery to reduce the pressure in the eye. It may also be used during eye examinations to reverse the effect of other eye drops used to dilate the eye.

    Oral Dry mouth after radiotherapy for malignant neoplasms of the head and neck

    Adult: Initial dose: 5 mg tid. If necessary, increase dose gradually after 4 wk until adequate response is achieved. Max dose: 10 mg tid. Oral Sjogren's syndrome

    Adult: 5 mg 4 times daily. Max: 30 mg daily. Stop treatment if no improvement after 3 mth. Hepatic impairment: Reduce dose in moderate to severe cirrhosis. Recommended dose: Child pugh category B: 5 mg bid. Child pugh category C: Avoid usage. Ophthalmic Open-angle glaucoma

    Adult: As HCl or nitrate: Instill a 0.5-4% soln into the eye 4 times daily. As modified-release preparation: insert preparation into the conjunctival sac which releases 20-40 mcg/hr for 7 days. As 4% ophth gel: apply once nightly into the affected eye(s). Hepatic impairment: Reduce dose in moderate to severe cirrhosis. Recommended dose: Child pugh category B: 5 mg bid. Child pugh category C: Avoid usage.

As modified release preparation which releases 20-40 mcg/hr for 7 days: Insert into the conjunctival sac. As 4% gel: Apply at bedtime.

    Pilocarpine is a muscarinic alkaloid obtained from the leaves of tropical American shrubs from the genus Pilocarpus. It is a non-selective muscarinic receptor agonist in the parasympathetic nervous system, which acts therapeutically at the muscarinic acetylcholine receptor M3 due to its topical application, e.g., in glaucoma and xerostomia.
