Propranolol - Used for: Treating certain types of irregular heartbeat. It may also be used for other conditions as determined by your doctor.

Propranolol is a beta-blocker. It works by decreasing the action of pacemaker cells and slowing certain impulses in the heart. This helps to control irregular heartbeat.

Propranolol is a non-selective beta-blocker. This medication has antihypertensive, antianginal and antiarrhythmic action.

The hypotensive effect is associated with a decrease in minute volume of blood, sympathetic stimulation of peripheral vascular disease, decreased activity of the renin-angiotensin system (has a value in patients with initial hypersecretion of renin), sensitivity of baroreceptors of the aortic arch (not going to enhance their activity in response to decreased blood pressure) and the effect on the CNS. The hypotensive effect is stabilized by the end of the second week of the prescription course.

The antianginal effect of Propranolol is due to reduced myocardial oxygen demand (due to the negative chronotropic and inotropic effect).

The antiarrhythmic effect is due to the elimination of arrhythmogenic factors (tachycardia, increased activity of the sympathetic nervous system, increase of cAMP, hypertension), decrease in the rate of spontaneous excitation of the sinus and ectopic pacemakers and slowing of AV-holding.

The ability of this drug to prevent the development of headache of vascular origin is due to a decrease in expression of the expansion of cerebral arteries due to beta-adrenoblockade vascular receptors, inhibition induced by catecholamines platelet aggregation and lipolysis, reduction of platelet adhesiveness, prevention of activation of coagulation factors during the release of adrenaline, the stimulation of oxygen supply to tissues and decreased secretion of renin.

Reduction of tremor during treatment with Propranolol caused mainly by the blockade of peripheral beta2-adrenergic receptors.

This medicine increases the atherogenic properties of blood; strengthens uterine contractions (spontaneous or induced by means of stimulating the myometrium); increases the tone of the bronchi; in high doses cause sedation.

Propranolol, the prototype of the beta-adrenergic receptor antagonists, is a competitive, nonselective beta-blocker similar to nadolol without intrinsic sympathomimetic activity. Propanolol is a racemic compound; the l-isomer is responsible for adrenergic blocking activity.

Propranolol helps to keep your blood pressure under control. It may also be used to treat certain heart diseases such as angina pectoris (chest pain) and arrhythmia (irregular heart beats). Propranolol may also be used to relieve palpitations (fast and irregular heartbeats) commonly experienced by people with an overactive thyroid gland. Propranolol is also used together with other medicines to prevent migraine or in the treatment of specific complications caused by liver disease.

Oral Hypertension

Adult: As conventional tablet or oral solution: Initially, 40-80 mg bid. Usual range: 160-320 mg daily. As extended release tablet: Initially, 80 mg once daily. Usual range: 120-160 mg once daily. Max: 640 mg daily.

Child: Initially, 1 mg/kg daily in divided doses, may increase to 2-4 mg/kg daily in 2 divided doses. Oral Phaeochromocytoma

Adult: 60 mg daily given on the 3 days before the operation with α-blockade. Prolonged treatment of 30 mg daily may be given if tumour is inoperable.

Child: 250-500 mcg/kg 3-4 times daily. Oral Myocardial infarction

Adult: 40 mg 4 times daily for 2-3 days followed by 80 mg bid. Alternatively, 180-240 mg daily in divided doses. Doses to be given within 5-21 days of MI. Oral Cardiac arrhythmias

Adult: 30-160 mg daily in divided doses.

Child: 250-500 mcg/kg 3-4 times daily. Oral Prophylaxis of migraine

Adult: Initially, 40 mg bid-tid increased to 160 mg daily. Up to 240 mg daily may be necessary. Discontinue therapy if satisfactory response not obtained within 4-6 wk after reaching max dose.

Child: <12 yr: 20 mg bid-tid. Oral Portal hypertension

Adult: Initially, 40 mg bid increased up to 160 mg bid. Oral Angina pectoris

Adult: As conventional tablet or oral solution: 40 mg bid-tid, may increase to 120-240 mg daily, up to 320 mg daily may be required in some patients. As extended release tablet: 80 mg once daily, increased as needed every 3-7 days, average dose 160 mg once daily, max: 320 mg daily. Oral Hypertrophic cardiomyopathy

Adult: 10-40 mg, given 3-4 times daily. Oral Hyperthyroidism

Adult: 10-40 mg, given 3-4 times daily. Oral Anxiety

Adult: 40 mg once daily, may increase to bid-tid. Oral Essential tremor

Adult: 40 mg bid-tid. May increase at wkly intervals to 160 mg daily. Up to 320 mg daily may be necessary. Intravenous Emergency treatment of cardiac arrhythmias

Adult: 1 mg injected over a period of 1 minute, repeated every 2 minutes, if needed. Max: 10 mg in conscious patients and 5 mg in patients under anaesthesia.

Child: 25-50 mcg/kg via slow inj, may be repeated 3-4 times daily. Intravenous Hyperthyroidism

Adult: 1 mg, given over 1 minute, may repeat at 2-minute intervals until response is observed or a max of 10 mg is used in conscious patients or 5 mg in patients who are under anaesthesia.

MI 40 mg 4 times/day for 2-3 days, then 80 mg twice daily. To be given w/in 5-21 days of MI. Portal HTN Initial: 40 mg twice daily, up to 160 mg twice daily. Angina pectoris 40 mg 2-3 times/day, up to 120-240 mg/day. Extended release 80 mg once daily, increase as needed. Max: 320 mg/day. Hypertrophic cardiomyopathy 10-40 mg 3-4 times/day. Hyperthyroidism 10-40 mg 3-4 times/day. Anxiety 40 mg once daily, may increase to 2-3 times/day. Essential tremor 40 mg 2-3 times/day. May increase wkly. Up to 320 mg/day if needed. IV Emergency treatment of cardiac arrhythmias; Hyperthyroidism 1 mg, repeat every 2 mins if needed. Max: 10 mg in conscious patients and 5 mg in patients under anesth.

Propranolol is a non-selective beta blocker mainly used in the treatment of hypertension. It was the first successful beta blocker developed. It is the only drug proven effective for the prophylaxis of migraines in children. Propranolol is available in generic form as Propranolol hydrochloride, as well as an AstraZeneca and Wyeth product under the trade names Inderal, Inderal LA, Avlocardyl (also available in prolonged absorption form named "Avlocardyl Retard"), Deralin, Dociton, Inderalici, InnoPran XL, Sumial, Anaprilinum (depending on marketplace and release rate)
