Salbutis - Beta adrenoagonists with a predominant effect on beta2-adrenergic receptors (localized, particularly in the bronchi, myometrium, blood vessels).

This medication prevents and reduces or eliminates bronchospasm, reduces the resistance in the airways, increases the vital capacity. It prevents the release of histamine, slow reacting substance from mast cells and factors chemotaxis of neutrophils. Compared with other drugs of this group has a less pronounced positive chrono-and inotropic effect on myocardium. It widen of coronary arteries, practically does not reduce blood pressure. Has tocolytic effect, lowering the tone and the contractile activity of the myometrium.

Salbutis (INN) or albuterol (USAN), a moderately selective beta(2)-receptor agonist similar in structure to terbutaline, is widely used as a bronchodilator to manage asthma and other chronic obstructive airway diseases. The R-isomer, levalbuterol, is responsible for bronchodilation while the S-isomer increases bronchial reactivity. The R-enantiomer is sold in its pure form as Levalbuterol. The manufacturer of levalbuterol, Sepracor, has implied (although not directly claimed) that the presence of only the R-enantiomer produces fewer side-effects.

Child: 1 mth-2 yr: 100 mcg/kg (max: 2 mg), 2-6 yr: 1-2 mg, >6 yr: 2 mg. Doses to be taken 3-4 times daily.

Elderly: Initially, 2 mg 3-4 times daily. Inhalation Acute bronchospasm

Adult: As aerosol: 100 or 200 mcg (1-2 puffs) 3-4 times daily. 2 puffs may be given prior to exertion to prevent exercise-induced bronchospasm. Inhalation Acute severe asthma

Adult: As MDI: 4-6 inhalations may be given every 10-20 min via a large volume spacer. Parenteral Severe bronchospasm

Adult: 250 mcg (as a solution of 50 mcg/ml) via IV inj, or via IV infusion of a solution containing 5 mg in 500 ml at a rate of 3-20 mcg/min adjusted according to patient\'s need. Higher dosages may be used in respiratory failure. IM/SC: 500 mcg, repeated every 4 hr if necessary. Intravenous Uncomplicated premature labour

Adult: For arrest of preterm labor between 24 and 33 wk of gestation: Initially, 10 mcg/min using a dilute solution of 20 mcg/ml in glucose 5% (200 mcg/ml of Salbutis if using a syringe pump), increase rate gradually at 10-min intervals until there is response; then increase slowly until contractions cease. Maintain rate for 1 hr after contractions have stopped, then gradually reduce rate by 50% at intervals of 6 hr. Usual dose: 10-45 mcg/min. Avoid prolonged therapy. Inhalation Severe bronchospasm

Salbutis sulfate is usually given by the inhaled route for direct effect on bronchial smooth muscle. This is usually achieved through a metered dose inhaler (MDI), nebuliser or other proprietary delivery devices (e.g. Rotahaler or Autohaler). In these forms of delivery, the maximal effect of Salbutis can take place within five to twenty minutes of dosing, though some relief is immediately seen. Salbutis can also be given orally as an inhalant or intravenously.

Maintain rate for 1 hr after contractions have stopped, then gradually reduce rate by 50% at 6 hrly intervals. Usual: 10-45 mcg/min. Avoid prolonged therapy. IM/SC Severe bronchospasm 500 mcg 4 hrly if needed. Inhalation Acute bronchospasm As aerosol: 100 or 200 mcg 3-4 times/day. 2 puffs may be given prior to exertion to prevent exercise-induced bronchospasm. Acute severe asthma As MDI: 4-6 inhalations every 10-20 mins. Severe bronchospasm Via nebuliser: 2.5-5 mg up to 4 times/day.
