Valcyclor - Valcyclor (INN) or Valacyclovir (USAN) is a prodrug and synthetic purine nucleoside analogue with inhibitory activity against herpes simplex virus types 1 (HSV-1), 2 (HSV-2), varicella-zoster virus (VZV), Epstein-Barr virus (EBV), and cytomegalovirus (CMV). Valcyclor is almost completely converted to acyclovir and L-valine. The inhibitory activity of Valcyclor is highly selective due to its affinity for the enzyme thymidine kinase (TK) encoded by HSV and VZV. This viral enzyme converts acyclovir into acyclovir monophosphate, which is then converted into acyclovir diphosphate and triphosphate by cellular enzymes. Acyclovir is selectively converted to the active triphosphate form by cells infected with herpes viruses.

Valcyclor is an anti-viral medicine. It is used to treat or prevent herpes simplex virus infections such as cold sores and genital herpes. Valcyclor helps to reduce the number of blisters and helps them to heal more quickly. However, it cannot cure herpes infections. Valcyclor is also used to treat shingles. Valcyclor is also used to treat cytomegalovirus infection that may occur after a kidney transplant. In children, Valcyclor may also be used to treat chickenpox.

Oral Herpes zoster (shingles)

Adult: 1 g tid for 7 days; commence within 72 hr of rash. Renal impairment: Patients on haemodialysis: 1 g daily after haemodialysis. CrCl (ml/min) Dosage Recommendation 15-30 1 g bid. <15 1 g daily. Hepatic impairment: Use with caution. Oral Genital herpes

Adult: Initial episode: 1 g bid for up to 10 days, begin course within 72 hr of symptom onset. CrCl (ml/min) Dosage Recommendation 10-29 1 g once daily. <10 500 mg once daily. Hepatic impairment: Use with caution. Oral Prophylaxis of herpes simplex infections

Recurrent episodes: 500 mg twice daily for 5 days. Recurrent genital herpes 500 mg twice daily for 3 days. Suppression of recurrent episodes of genital herpes 1 g once daily or 500 mg once daily for patients w/ ≤9 recurrences per year; HIV infected patients with CD4 cells count ≥100 cells/mm 3 : 500 mg twice daily,up to 6 mth. Reduction of transmission of genital herpes W/ safe sex practices and abstinence: 500 mg once daily for infected partner w/ history of ≤9 recurrences per yr. Prevention of CMV infections in immunocompromised patients 2 g 4 times/day for 90 days w/in 72 hr after transplant. Herpes labialis 2 g12 hrly for 1 day.
